Please note, per licensure limitations, we can only accept patients who reside in Washington state.
A note to New Patients:
Use schedule below to request a new patient appointment.
You'll be directed to a short pre-screening form. After reviewing it, we’ll either approve your appointment or reach out to schedule a quick consult if needed.
A credit card is required to hold your spot. Please review our cancellation policy—your card may be charged for no-shows or late cancellations (less than 2 business days’ notice).
Once approved, you’ll get an invitation to create your ChARM account and complete your paperwork.
